Required Employment Documents

Prior to starting work in your first position through Student Employment, you must visit the Human Resources office located in Rivers Memorial Hall room 100 to complete the following form

*Federal I-9 Form

Please visit I-9 Documentation for the list of identification you will need to bring with you to complete the Federal I-9 Form.  You may not work until this form is completed and the proper identification is presented in Human Resources.

ALL DOCUMENTS MUST BE ORIGINAL AND UNEXPIRED. PHOTOCOPIES AND ELECTRONIC SUBMISSIONS ARE NOT ACCEPTABLE.

Name and Address Changes

It is imperative for the University to have correct information on students and employees.  Address changes may be completed in Self Service.  Name changes are done through the Enrollment Services department located below D’Amour Library.
